Senior Connection is a Jewish Family Service program which is committed to helping older adults age with dignity and respect. In assessing an older adult, Senior Connection matches resources to the adult’s needs. A care plan will be designed by a JFS care manager to monitor the appropriate delivery of services to the older adult, thus assuring the individual’s continuum of care.
What Services are Provided?
- Assessment/Evaluation:
- Identifies an older adult’s psychological and social functioning, physical abilities, living situation and safety needs.
- Development of Customized Care Plan:
- Assists the client and family to assure an enhanced quality of life by matching programs to each older adult’s unique needs. Services are organized to assure the individual’s safety, enhance the level of activity and maintain independence.
- Information and Referral/Assistance:
- Provides access to community resources that link you to a variety of services, including:
- Activities of Daily Living
- Counseling (licensed clinical social workers are prepared to meet a wide range of needs)
- Fitness and Recreation
- Hospital Discharge Planning
- Housing
- Meal Delivery
- Personal Affairs Management (checkbook balancing, bill paying)
- Shalom Taxi (provides safe and affordable subsidized travel for medical appointments, grocery shopping, personal care needs, or travel to and from Jewish institutions)
- Ongoing Communication with Client and Family:
- Through continuous dialogue with the older adult and his/her family, our Senior Care Manager can identify changes in the client, often before a problem develops.

- Older adults who need ongoing reassurance and evaluation because their families live out of town.
