enior Connection is a Jewish Family Service program which is committed to helping older adults age with dignity and respect. Senior Connection matches resources to the adult’s needs. A care plan will be custom designed by a JFS care manager to monitor the appropriate delivery of services to the older adult, thus assuring the individual’s continuum of care. This can include regular calls with the senior adult, setting up a schedule to ensure proper care, organizing transportation, and being at the senior adult's side in person when needed.

Frequent communication with the family and senior adult is essential! Through continuous dialogue with the older adult and his/her family, our Senior Care Manager can identify changes in the client, often before a problem develops.

When the social worker came to my house to visit me, I began to understand my daughter’s concerns; now I’m ready to make some changes.
